The doctors welcome new patients who live within our practice area. If you move out of the practice area you may need to register with another practice.

All new patients are invited to make an appointment to have a health check with our practice nurse.

Register With Us

To register with us, please check you live within our catchment area; you can do this using the map below.

Once you have ensured you live within our boundary, please complete the online ‘New Patient Registration’ form.

  • The practice accepts applications from all patients to join the practice regardless of race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition.

We encourage all patients to register securely online using the link provided above.

However, patients may also register as a new patient at Priors Field Surgery by following the steps below:

Please ensure you complete each section of the form fully in order for your registration to be completed. You should sign and date both forms, and ensure you include your NHS number, if you have one.

Patients should complete both forms for each member of the family being registered. If you are registering a new born child, please only complete the PRF1 form.

It is helpful to have proof of ID when you register with us as it can assist us in matching previous medical records and confirm parental responsibility. Examples of ID proof include passport, driving licence, birth certificate, HC2 certificate, rough sleepers’ identity badge or hostel or accommodation registration or mail forwarding letter.

If you are homeless, you can give a temporary address, such as a friend’s address, a day centre or the GP surgery address. But again it is helpful to have a mobile number so we have a means of communicating with you.

Ensure you complete every part of each registration form, especially the signature sections. Any gaps in information may result in a delay in your registration.

Please allow 10 working days for us to process your registration.

Once you have registered with us you will be allocated a named accountable GP. You can request to see any doctor. However, where possible, it is preferred that a patient sees the same doctor to encourage continuity of care.

Your health record & sharing of information

Your health record includes your medical history, details about your medication and any allergies you may have. With your permission, clinicians and others in healthcare services caring for you can share your record.

Giving permission to share your health record helps the NHS deliver the best level of care for you.

You have two choices which allow you to control how your record is shared. You can change these choices at any time by letting the surgery or service you are using know.

Your choices:

  • Sharing OUT – this controls whether the information recorded at this surgery or service can be shared with other healthcare services.
  • Sharing IN – this determines whether or not this surgery or service can view information in your record that has been entered by other services who are providing care for you, or who may provide care for you in the future.

Simply download and complete this form to indicate your sharing of your personal information choices.

Change of personal details

It is important that personal details on medical records are kept up-to-date. If you have changed your name, address, phone number or marital status, please do let the surgery know as soon as possible, so your records can be accurately maintained.

If your name has changed due to marriage or by Deed Poll, please provide a copy of the relevant documentation to be added to your records. If you have changed address, please check if you are still within our practice area boundary.

You can also update your next of kin, carer details or responsibilities, and other changes, such as Power of Attorney details, too. We will happily record these changes to your medical record. If you have online access to your record – via SystmOnline or the NHS App – you can update your personal details yourself, otherwise please contact the surgery.

If you move away from our practice boundary area, you have 30 days to register with a new GP practice before we deduct your medical records.

Temporary Patient Registrations

If you are not registered with the surgery you will be unable to see one of our doctors unless you have a serious emergency. In most other cases, calling 111 is a better option.

If you are not registered but need treatment at the surgery you will need to complete a temporary registration form.

You can be registered as a temporary patient for up to three months. This means you can be registered with our practice and remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with our practice.

Details of any treatment you have will be passed  to your permanent GP surgery. They will add the information to your medical records.

Overseas Visitors

Those who visit the UK from overseas must pay for their care when they are in England.

In 2015 changes were made to how the NHS charges overseas visitors for healthcare so the NHS does not lose out on income for the services these patients have received.

Find Your NHS Number

Sometimes when filling in online forms or speaking with our staff, you may be asked for your NHS number.

This online tool will help you find your number if you do not know it.

Please Note

You do not need to know your NHS number to use NHS services, but it can be useful to have it.

Other ways to get your NHS number

If you cannot get your NHS number online you can:

  • find it on any letter from the NHS like a prescription or appointment letter
  • call your GP surgery and ask them for your number
